the position of MDS Nurse. As a member of our leadership team, you
will play a crucial role in nurturing an environment that

responsible for ensuring that resident assessments are not only
accurate but also reflective of optimal care strategies. This vital
role encompasses a range of responsibilities such as:
- Overseeing the initiation and completion of resident
assessments (MDS) in strict accordance with current regulations and
guidelines.
- Guaranteeing the timely and precise execution of the MDS
process.
- Cooperating with an interdisciplinary team to craft
personalized care plans for each resident.
- Tracking and documenting residents' health status and their
reactions to care plans.
- Engaging in the Quality Assurance and Performance Improvement
(QAPI) initiatives.
- Ensuring clear communication between residents, their families,
and the healthcare team to facilitate seamless care delivery.
- Staying informed regarding MDS regulations and guidelines to

annually Qualifications:
- Must hold a current RN (Registered Nurse) license.
- RAC-CT (Resident Assessment Coordinator-Certified)
certification is required.
- In-depth understanding of MDS 3.0, RAI processes, and
individualized care planning.
- Strong interpersonal and organizational skills are
essential.
- Ability to operate independently while also thriving in a team
environment.
- A sincere commitment to delivering high-quality care to all
residents. This is a full-time, on-site position.
