The MDS Nurse is responsible for ensuring that resident assessments are not only accurate but also reflective of optimal care strategies. This vital role encompasses a range of responsibilities such as:

• Overseeing the initiation and completion of resident assessments (MDS) in strict accordance with current regulations and guidelines.
• Guaranteeing the timely and precise execution of the MDS process.
• Cooperating with an interdisciplinary team to craft personalized care plans for each resident.
• Tracking and documenting residents' health status and their reactions to care plans.
• Engaging in the Quality Assurance and Performance Improvement (QAPI) initiatives.
• Ensuring clear communication between residents, their families, and the healthcare team to facilitate seamless care delivery.

    • Must hold a current RN (Registered Nurse) license.
    • RAC-CT (Resident Assessment Coordinator-Certified) certification is required.
    • In-depth understanding of MDS 3.0, RAI processes, and individualized care planning.
    • Strong interpersonal and organizational skills are essential.
    • Ability to operate independently while also thriving in a team environment.
