Description:Zepf Center has been serving the Lucas County
community for nearly 50 years. We are the leading provider of
behavioral health and substance use disorder services in Northwest
Ohio. Services include adult and child psychiatric, substance
abuse, case management, residential, Crisis Care, and therapy
programs, as well as career development and wellness services. Zepf
Center also offers primary care medical services to our patients to
contribute to their continuum of care. Zepf Center is a
trauma-informed agency and environment for both patients and
staff.
We are currently seeking a Full-Time Case Manager. The purpose of
this position is to provide a wide range of community support and
coordination of services to people experiencing serious mental
illness to ensure their needs are addressed. The ACT Case Manager
is accountable for carrying out the case management processes of
problem identification, assessment, planning care, interventions,
evaluation of client response, and revision of plan of care, as
appropriate, in accordance with ACT fidelity requirements. The Case
Manager works with the client in the community, home environment,
and at agency locations. Works hours/shifts as posted.

WORKING CONDITIONSThis job is a fast-paced position that include a
high volume of individual and family contact, collaboration with
various community agencies, as well as computer entry and tracking
and requires cultural/racial diversity and sensitivity. Employee
must be able to maintain a patient engagement standard.
Employee's work is conducted in offices, participant's homes, and
other community sites. Employee has contact with a wide range of
people including some exhibiting anger and frustration under
stressful conditions. Participants may be drug or alcohol-affected
and in need of treatment. The employee will be exposed to and need
to deal with a range of emotionally charged issues in a
compassionate, trauma-informed manner.
Must have a valid driver's license and acceptable driving
record.
