We are in need of a Full Time Travel Physical Therapist. The schedule will run as four 10-hour shifts Tuesday through Friday.$10,000 Sign-On BonusOur Care Coordination team provides our therapist with valuable resources such as scheduling and documentation support so that you are able to focus on what matters most, patient care.ProHealth Partners is committed to providing the highest standards of Physical, Occupational and Speech therapy for patients in their homes. With our corporate office located in Perrysburg, Ohio, we currently service Northwest Ohio, the Greater Cleveland, Ohio area, as well as Southeast Michigan. We place a strong value not only on the patients and the agencies we work with but our employees as well.Job Responsibilities:Supervision to Asst. therapy staffCommunicating daily with office staff to accept new patients, complete paperwork and turn in payrollKeeping open communication with patients and home health agencies to promote a positive and caring experienceFacilitating communication with Physical Therapist Assistant to manage patient careCompletion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperworkAttend meetings and case conferencesJob Requirements:Current Physical Therapist Licensure in MichiganBachelor's degree in Physical TherapyAbility to travel to patient's home or facilityStrong communication skillsHome healthcare experience preferredsome weekend on call rotationSalary options and full time per visit rates are negotiable
